Transfer Coordinator Registered RN

The Nurse Transfer Coordinator (TC) Registered Nurse (RN) is responsible for the oversight and coordination as it pertains to patient movement and referrals to and from the facility. The nurse demonstrates proficient practice and decision-making, deliberate planning, and critical thinking skills. The TC RN demonstrates the ability to manage complex patient care for patients receiving care through all designated community and Veterans Affairs (VA) tertiary sites
