Description
As a team member of the HUD-VASH program, the HUD-VASH Registered Nurse Care Manager is a member of the HUD-VASH program and as such will provide support to the HUD-VASH teams and serve in a medical and mental health consultative role. The incumbent consults with the Lead HUD-VASH case manager, the HUD-VASH teams, the Medical Director for the Homeless Patient Aligned Care Team, the VISN Homeless Coordinator and other colleagues as needed to best meet the treatment needs of the Veteran.
