Description

Position Summary The Health Services Director (HSD) oversees the Assisted Living (AL) and Memory Care (MC) program at Maple Tree Village and Greenhaven Terrace including, but not limited to nursing care aspects, direction, orientation, training and supervision of staff, physical assessments of residents, communication with physicians and other medical care professionals and families. The HSD assists with staff development and works with all levels of staff to ensure that specialized memory care programs are implemented. This position is exempt and reports directly to the Executive Director (ED). Essential Job Functions Coordinates implementation of AL & MC program mission, goals, & philosophy. Works closely with the ED and Sales Counselor in reviewing and/or performing the pre-move-in assessments to evaluate eligibility for move-in. Maintains communication with health care professionals at acute, post-acute, SNF communities while current ACC residents are under their care. Conducts reassessments for return eligibility, ensuring appropriate and timely return to the community. Conducts ongoing routine assessments for all residents, and nursing assessments in response to change of condition identified by staff. Remains alert to the residents’ condition and reports needs, changes, and concerns to the Executive Director, resident RP, physician and other community health professionals, as appropriate. Participates and leads service plan meetings and ensures timely services plans are implemented for every resident in accordance with current rules, regulations, and community policies and procedures that govern resident assessments. Ensures that the provision of services is in accordance with the physician’s orders and that the resident’s medical needs are met. Supervises the resident care team members to ensure that all aspects of care are delegated and completed, medications are distributed in a timely and accurate manner as per RCFE regulations; resident service plans are being carried out appropriately, residents are clean, well groomed, well nourished; residents are participating in activities of choice; residents are comfortable. Responsible for unusual incident reporting and ensures any unusual incidents are reported timely to the resident’s RP, ED and CCL. Supervises ordering of medications and medical supplies. Supervises the transcription of medical orders, and all other areas of documentation in medical records. Supervises audits of all charts for compliance; closes charts for residents discharged. Conducts daily charting reviews and 24-hour log monitoring for follow-up. Conducts routine and change of condition care conferences; assists with direct resident care as needed. Manages monthly/annual long-term care insurance paperwork. Holds regular/monthly department staff meetings; ensures sufficient and adequate staffing levels are maintained through proactive employee scheduling to ensure resident care needs are met at all times. Assists with orientation, staff training and all aspects of daily care. Develops a schedule to promote regular orientation and in-service education, and one-on-one staff development as needed. Qualifications QUALIFICATIONS: California licensed Registered Nurse (RN) or a California Licensed Vocational Nurse (LVN) is required. Minimum two years in a leadership/management role, leading clinical staff in a senior living environment Knowledge of RCFE allowable, restricted and prohibited conditions; procedures for requesting exceptions and waivers and requirements for reporting unusual conditions. Minimum two plus years of work-related experience working with dementia and/or Alzheimer’s clients; knowledgeable about the disease process, changes associated with aging and dementia, how they affect the individual’s ability to function, and the adaptive strategies that help maintain a resident’s abilities. Knowledge of the requirements for providing care and supervision appropriate to the residents and for the person living with dementia. Effective English communication skills, both written and verbal; bilingual is a plus. RCFE Administrators Certificate a plus.
