We are seeking a dedicated Home Health Registered Nurse (RN) Case Manager to manage a defined territory within Luzerne County. This role is 100% patient care focused and ideal for a nurse who values autonomy, continuity of care, and building strong relationships with patients in the home setting.
Each RN Case Manager is assigned their own territory and collaborates with teammates to provide coverage during scheduled time off.
Schedule & Work Details
• Schedule: Monday-Friday
• Hours: 8:00 AM - 4:30 PM
• On‑Call Requirements:
• Every 3rd weekend
• One night of on‑call per week
• Mileage Reimbursement: Yes — reimbursed between visits, from first to last patient of the day
• Dress Code: Scrubs (no specific color requirement)
Key Responsibilities
• Serve as RN Case Manager for an assigned patient territory
• Perform skilled nursing visits including:
• Routine skilled visits
• Start of Care (SOC) visits
• Resumption of Care (ROC) visits
• OASIS recertifications
• Patient discharges
• Conduct comprehensive nursing assessments and monitor changes in patient condition
• Develop, implement, and update individualized plans of care
• Coordinate care with physicians, therapists, and interdisciplinary team members
• Provide patient and caregiver education to support safe care at home and prevent hospital readmissions
• Accurately complete clinical documentation in accordance with regulatory and agency standards
• Travel daily to patient homes within the assigned territory
• Provide cross‑coverage for fellow case managers during PTO or scheduled absences
Required Qualifications
• Active Registered Nurse (RN) license
• Ability to independently manage a patient caseload and territory
• Strong clinical assessment and time‑management skills
Preferred Qualifications (Nice to Have)
• Homecare Homebase (HCHB) experience
• Recent home health nursing experience
• Experience completing OASIS documentation
• Strong communication skills and patient‑centered approach to care
